A new analysis of California health data has highlighted the alarming scale of the problem, showing that the number of confirmed silicosis cases among engineered-stone countertop workers has risen dramatically in recent years.
California health officials identified 592 cases of silicosis among workers involved in quartz countertop fabrication between January 2019 and June 2026. At least 31 of those workers have died, while another 65 have required lung transplants.
The figures underscore the serious health consequences associated with cutting, grinding and polishing engineered stone.
Silicosis is caused by breathing respirable crystalline silica, a fine dust released when stone products are cut or processed. The disease causes inflammation and scarring in the lungs and is progressive, irreversible and incurable.
Engineered stone, often marketed as quartz, can contain more than 90% crystalline silica, making workers who fabricate the material particularly vulnerable when adequate dust controls are not used.
California officials have been tracking the problem since 2019, when an initial case involving a countertop worker led investigators to discover additional illnesses among employees at the same workplace.
Since then, the number of identified cases has increased sharply.
The latest data indicate that the outbreak is affecting workers at a relatively young age compared with many traditional occupational lung diseases.
The median age at diagnosis among affected California countertop workers is 46, while the median age at death is 52.
Men account for approximately 99% of the reported cases, and about half of the cases have occurred in Los Angeles County, where the state’s countertop fabrication industry has a significant presence.
The disease can develop after workers repeatedly inhale microscopic silica particles released during fabrication.
When the particles reach deep into the lungs, the body attempts to respond to the foreign material, triggering inflammation and eventually permanent scarring.

A 2023 study of California immigrant engineered-stone workers found that many patients experienced delays in receiving the correct diagnosis. Researchers said some were initially diagnosed with conditions such as bacterial pneumonia or tuberculosis.
The consequences can be devastating.
In one early California case, a worker developed severe silicosis after years of fabricating engineered stone and died at just 38 years old.
Other affected workers have also required transplants while still relatively young.
The growing number of illnesses has prompted calls for stronger workplace protections and potentially stricter limits on engineered stone.
California regulators have already taken enforcement action against fabrication businesses found to be violating workplace safety requirements.
The central concern is the amount of silica dust workers inhale during cutting and finishing operations.
Using water during cutting can help suppress dust, while ventilation systems, specialized machinery and properly fitted respiratory protection can further reduce exposure.
However, public health researchers say existing protections have not prevented the continued emergence of new cases.
