
Tennessee Prison Chief to Resign After Failed Christa Pike Execution
Frank Strada, commissioner of the Tennessee Department of Correction, will leave his position later this month. His resignation comes days after Pike survived a lethal injection attempt that left her unconscious and in critical condition.
The failed execution has prompted an independent review and a temporary halt to executions in Tennessee for the rest of 2026.
Frank Strada to Leave Tennessee Prison System
Governor Bill Lee said Strada offered his resignation and that he accepted it.
Strada said stepping down was in the best interests of Tennessee while the independent investigation continues. He also said he believes correction department employees followed the established execution protocol.
Strada became commissioner of the Tennessee Department of Correction in January 2023. He oversees the state’s prison system, including the procedures used for executions.
His departure is the latest development following the failed attempt to execute Pike at Riverbend Maximum Security Institution in Nashville.
Christa Pike Execution Failed
The execution did not proceed as planned after officials administered two doses of pentobarbital. Pike became unconscious, but she remained alive.kMedical staff have been working to treat her condition and clear the execution drug from her system.
The incident has raised questions about the execution process, particularly the placement of intravenous lines used to administer the drugs.
Pike’s lawyers have alleged that the IV lines were not correctly placed and that some of the drug entered tissue instead of her bloodstream. Those allegations are part of the ongoing review.
Independent Review Ordered
Governor Lee has ordered an independent review of the failed execution.
The investigation will also consider the medical and technical problems that occurred during the lethal injection.
Lee has halted all remaining executions scheduled in Tennessee for the rest of 2026 while the review takes place.
The state has not announced when executions will resume.
Pike Remains in Critical Condition
Pike remained in critical condition following the failed execution, according to her attorneys.

Pike’s attorneys had previously raised concerns about her medical condition. They pointed to thrombocytosis, a blood disorder, and difficulties with establishing IV access.
Pike Was Sentenced to Death in 1995 Case
